Coaching Agreement / Participant Waiver

Coach Name (Optional in Delivery, Not in Agreement): Dr. Shane Cope

 

1. Parties

This Agreement is entered into by and between Essentials Health Ventures, LLC ("Company," "we," or "us") and [Participant Name] ("Client" or "You"). By enrolling in and participating in the Dry Mouth & Burning Mouth Recovery Roadmap™ Group Coaching Program ("Program"), you agree to the terms outlined below.

 

2. Nature of Coaching Services

You understand that the Program offers group coaching services that are educational and informational in nature.

• These services are not medical advice.
• Participation in this Program does not create a doctor-patient relationship with Dr. Shane Cope or Essentials Health Ventures, LLC.
• No individualized diagnosis, treatment, or medical care will be provided.

 

3. Not Medical or Mental Health Advice

You understand and agree that:

• All information shared in the Program (live sessions, digital materials, or any other format) is for informational and educational purposes only.
• This Program is not a substitute for professional medical or mental health advice, diagnosis, or treatment.
• You are responsible for consulting your own licensed healthcare providers before making any health-related decisions or changes.

 

4. Personal Responsibility

By participating, you acknowledge and agree:

• You are solely responsible for your own physical, mental, and emotional well-being.
• You assume full responsibility for any decisions, actions, or results you choose to make based on your participation in the Program.
• Essentials Health Ventures, LLC makes no guarantees about specific outcomes or results.

 

5. Confidentiality

We will respect the confidentiality of any personal information shared in the group coaching sessions, with the exception of:

• Information you permit to be shared publicly
• Information required by law to disclose
  Please respect the privacy of other participants in the Program and keep their personal information confidential.

 

6. Group Coaching Environment

You understand that:

• Group coaching is a shared space, and there is no guarantee of confidentiality among participants.
• You will participate respectfully, maintaining professionalism in communications.
• The Company reserves the right to remove participants from sessions or the Program at its discretion for disruptive or inappropriate behavior.

 

7. Intellectual Property

All materials provided (videos, transcripts, PDFs, handouts, etc.) are the property of Essentials Health Ventures, LLC and are for your personal use only.
You agree not to:

• Copy, share, reproduce, distribute, or sell any materials without express written permission. 

 

8. Payment & Refund Policy

• Payment for the Program is due in full at the time of enrollment (or according to any agreed-upon payment plan).
• Refunds, if any, are subject to the terms listed on the product sales page or checkout. If no refund is specified, all sales are final.
• Missed coaching sessions are not eligible for refunds or rescheduling.

 

9. Release of Liability

You voluntarily assume all risks associated with your participation in the Program.
You hereby release and discharge Essentials Health Ventures, LLC, its owners, officers, employees, contractors, and affiliates from any and all claims, demands, or causes of action arising out of your participation in the Program.

This includes (but is not limited to):

• Personal injury
• Property damage
• Loss of income
• Emotional distress

 

10. Indemnification

You agree to indemnify and hold harmless Essentials Health Ventures, LLC and its affiliates from and against any claims, damages, liabilities, costs, or expenses (including legal fees) resulting from your participation or actions related to the Program.

 

11. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the state of [Insert State], without regard to conflict of law principles.

 

12. Entire Agreement

This Agreement represents the entire understanding between you and Essentials Health Ventures, LLC regarding participation in the Program and supersedes any prior agreements or understandings.

By enrolling in the Program and/or participating in group coaching sessions, you confirm that you have read, understood, and agreed to the terms of this Coaching Agreement and Participant Waiver.
